**The following notes were located in the personal database of Thomas Joseph Helms, whom is deceased.

Ambrose, age 30, voluntarily enlisted in the Confederate States Army on 09 June 1861 at Fort Smith. He was mustered as a Private in Company E (Capt. McAlexander's Company, 1st Arkansas Mounted Rifles (Churchill's Regiment). He was discharged on 15 July 1861.

He enlisted again in the C.S.A on 12 July 1862 at Smithville (Lawrence County), Arkansas and was mustered as a Private in Company D, 38th Arkansas Infantry. He was left sick at a Fort Smith hospital on 20 December 1862. According to roll call of 30 June-31 August 1863 he was dropped from the regimental roll.

Lastly, Ambrose was conscripted in the C.S.A. of Company E, Coffee's Regiment, Missouri Cavalry on 14 June 1864 as a 2nd Lieutenant. On his compiled military service record (CMSR) his name is sometimes listed as Andrew W. Smith. Not sure if gave his captures a false name or whether they misunderstood him. He was captured on 23 October 1864 at Independence, Missouri and sent to Military Prison in Fort Leavenworth, Kansas where he was noted sick in post hospital on 28 November 1864. On 27 December 1864 he was transferred to Johnson's Island, Ohio where he arrived on 28 December 1864. On 01 February 1865 at Johnson's Island, Ohio, he applied for the Amnesty Oath. The following statement was made by him: "Was born in Indiana. Says that he voluntarily enlisted as a private in June 1861 in which capacity he served about one month and was then discharged on account of disability. That he returned home and remained until June 14, 1864 when he was conscripted and immediately elected 2nd Lieutenant of Co. E, Coffee's Ark Regt of Cavy which position he held about two weeks and was then promoted 1st Lieut. and held this rank at the time of capture; that he always thought the South to hasty in secedeing; that his object in wishing to take the Amnesty Oath is to go home. Health is very poor." According to his CMSR he took the Oath of Amnesty on 27 March 1865 at Johnson's Island, Ohio. The following info was noted on his amnesty oath: Residence - Powhatan, Ark; Light complexion; Light hair; Blue eyes; 5 feet 11 inches tall; captured at Independence, Missoui Oct. 23, 1864. Not sure whether he truly took Oath of Amnesty because there is a Oath of Allegiance record, dated 16 June 1865.
